For over ten years, Cordaid staff in the Democratic Republic of the Congo (DRC) have been doing this successfully, with the support of the <a href=\"https:\/\/www.theglobalfund.org\/en\/\" target=\"_blank\" rel=\"noreferrer noopener\">Global Fund to Fight AIDS, Tuberculosis and Malaria<\/a>. This involved a vast logistics chain from procurement to distribution at the local level, right into the hands of patients, often in very challenging contexts.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Conflict and lack of infrastructure<\/h2>\n\n\n\n<p>&#8216;This is a country of extremes,&#8217; Fenneke Hulshoff Pol says. Fenneke has been supporting the team in the DRC from the Cordaid office in The Hague since 2017 and frequently travels to the programme&#8217;s locations. &#8216;The context varies dramatically depending on where you are. Apart from a few stretches of asphalted roads, constructed by foreign investors in recent years, most pathways through the dense forests are unpaved and virtually inaccessible in the rainy season.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>&#8216;We need to empower the country to take ownership of these health interventions.&#8217;<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\">Constant crisis mode<\/h2>\n\n\n\n<p>&#8216;Getting supplies to the health centres can take weeks,&#8217; Fenneke explains. &#8216;We tried to reach every part of the country by creating clusters and deploying teams to remote areas, but with COVID-19 on top of everything, it felt like we were in a constant crisis mode. The project structure had to evolve continuously to adapt to the new challenges.&#8217;<\/p>\n\n\n\n<p>The deputy project director for the programme in the DRC, Blaise Mudekereza, says the country&#8217;s bureaucracy also added to the programme&#8217;s operational complexity. &#8216;The administrative processes can be cumbersome. For example, every month, we must submit identity documents, contracts, and payment records, even for staff we\u2019ve worked with for years. This takes time and resources that could be better spent on patient care.&#8217;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Sustainability<\/h2>\n\n\n\n<p>In addition to streamlining bureaucracy, Blaise believes that the parties involved in the programme&#8217;s continuation should make sustainability a top priority. &#8216;We need to empower the country to take ownership of these health interventions. Currently, the government contributes less than 5% of the funding for these programmes. If external funding were to decrease, the country would struggle to maintain the progress we\u2019ve made. Building local capacity, especially in procurement and healthcare delivery, is essential for the future.&#8217;<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>&#8216;We ensured that life-saving medication got to people who otherwise wouldn\u2019t have had access to it.&#8217;<\/p>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\">Increasing responsibilities<\/h2>\n\n\n\n<p>After four fruitful years, the project activities expanded and the budget ballooned from around $160 million to over $230 million in 2018. A project of this size and scale comes with great responsibility and, at times, a ridiculous workload for the team, Fenneke recalls. Still, she looks back on her work for the programme with pleasure and pride. &#8216;We managed to reach a lot of people, particularly in the key populations.&#8217;<\/p>\n\n\n\n<p>These are groups with an increased risk of being affected by HIV and TB: sex workers, men who have sex with men, transgender individuals, and prisoners. People who are also often stigmatised and have very limited access to health care.<\/p>\n<\/div><\/div><\/div><\/div><\/div>\n\n\n\n<div class=\"section section--grey\">\n\t<div class=\"container\">\n\t\t<div class=\"block indicators\">\n\t\t\t<h2 class=\"screen-reader-text\">Results &amp; Indicators<\/h2>\n\t\t\t<ul class=\"indicators__list\">\n\t\t\t\t\t\t\t\t\t<li class=\"indicators__item\">\n\t\t\t\t\t\t<div class=\"indicator\">\n\t\t\t\t\t\t\t<div class=\"indicator__icon\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"icon icon-health-system\" aria-hidden=\"true\"><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<p class=\"indicator__content\">\n\t\t\t\t\t\t\t\t<span class=\"indicator__number\">18,059,638<\/span>\t\t\t\t\t\t\t\tHIV tests carried out in the programme between 2017 to 2023 \n\t\t\t\t\t\t\t<\/p>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/li>\n\t\t\t\t\t\t\t\t\t<li class=\"indicators__item\">\n\t\t\t\t\t\t<div class=\"indicator\">\n\t\t\t\t\t\t\t<div class=\"indicator__icon\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"icon icon-health-system\" aria-hidden=\"true\"><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<p class=\"indicator__content\">\n\t\t\t\t\t\t\t\t<span class=\"indicator__number\">4,000<\/span>\t\t\t\t\t\t\t\tHealth facilities provided with tests and medication up to 4 times a year\n\t\t\t\t\t\t\t<\/p>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/li>\n\t\t\t\t\t\t\t\t\t<li class=\"indicators__item\">\n\t\t\t\t\t\t<div class=\"indicator\">\n\t\t\t\t\t\t\t<div class=\"indicator__icon\">\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<span class=\"icon icon-health-system\" aria-hidden=\"true\"><\/span>\n\t\t\t\t\t\t\t\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t\t\t\t<p class=\"indicator__content\">\n\t\t\t\t\t\t\t\t<span class=\"indicator__number\">180,000<\/span>\t\t\t\t\t\t\t\tmen who have sex with men reached with testing and sensitisation between 2018 and 2023\n\t\t\t\t\t\t\t<\/p>\n\t\t\t\t\t\t<\/div>\n\t\t\t\t\t<\/li>\n\t\t\t\t\t\t\t<\/ul>\n\t\t<\/div>\n\t<\/div>\n<\/div>\n\n\n<div class=\"section section--core \"><div class=\"container\"><div class=\"row\"><div class=\"col-12 col-md-8 offset-md-2 block-wrapper\"><div class=\"entry-content\">\n<h2 class=\"wp-block-heading\">A safe space<\/h2>\n\n\n\n<p>They can now visit specialised centres, set up by the programme, where people get tested, receive treatment, and find legal support when needed. The centres also provide a safe space for people living on the fringes of society.<\/p>\n\n\n\n<p>Blaise: &#8216;We\u2019ve expanded the number of treatment sites for these groups from fewer than 5 to 25 over the last decade. Before our involvement, only 60 to 70% of people had access to HIV treatment. Today, over 90% of those diagnosed receive the medication they need.&#8217;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Diagnosis, treatments and extended care<\/h2>\n\n\n\n<p>When he reflects on the achievements his team and he have made over the past decade, three things stand out. &#8216;First, we\u2019ve significantly increased the number of people diagnosed with HIV and tuberculosis, ensuring they receive timely treatment. We\u2019ve also achieved a treatment success rate of over 90% for tuberculosis, meaning 9 out of 10 patients are fully cured after completing their treatment. And lastly, we\u2019ve successfully extended care to key populations, offering them medical, psychological, and legal support.&#8217;<\/p>\n\n\n\n<p>&#8216;We ensured that life-saving medication got to people who otherwise wouldn\u2019t have had access to it,&#8217; Fenneke adds. &#8216;I can\u2019t say health care in the DRC has transformed entirely over the past years, but we\u2019ve made a lot of progress.
